I understand that participation in classes, programs, rehearsals, performances, and activities at Rhymth and Unity Academy involves physical activity and inherent risks, including but not limited to injury, illness, falls, collisions, or other physical harm.
I voluntarily choose to participate (or allow my child to participate) and fully accept and assume all risks, both known and unknown, associated with participation.
I hereby release and hold harmless Rhymth and Unity Academy, its owners, instructors, staff, contractors, volunteers, and affiliates from any and all liability, claims, demands, or causes of action arising out of or related to any injury, illness, damage, or loss that may occur as a result of participation, including those caused by negligence to the fullest extent permitted by law.
I certify that I (or my child) am physically able to participate in dance and performing arts activities and have no medical condition that would prevent safe participation, or I have disclosed such conditions to the academy.
In the event of an emergency, I authorize Rhymth and Unity Academy to obtain medical treatment for me or my child if necessary. I understand that I am responsible for any medical expenses incurred.
I agree to follow all rules, instructions, and safety guidelines provided by Rhymth and Unity Academy staff. I understand that failure to follow these guidelines may result in removal from participation without refund.
I grant permission for Rhymth and Unity Academy to use photographs and/or video recordings of me or my child for promotional, marketing, and social media purposes without compensation.
If I do not wish for myself or my child to be photographed or recorded, I understand that I must submit this request in writing to rhythmandunity@gmail.com.
If signing on behalf of a minor, I certify that I am the parent or legal guardian and have the authority to agree to these terms.
